  • Urgency level (P0/P1/P2/P3; for response time see end of template)
  • Problem description
  • URL
  • Whether accessed from a terminal
  • Recurrence method

Definition of urgency levels:

  • P0: Immediate Processing
  • P1: Processing within 2 hours
  • P2: Processing within 1 day
  • P3: Processing within 3 days
